国产氯吡格雷联合国产药物洗脱支架在经皮冠状动脉介入治疗中的应用The application of domestic clopidogrel and domestic drug eluting stent in Pereutanous Coronary Intervention

摘要(Abstract)：
目的观察国产氯吡格雷(泰嘉)联合国产药物洗脱支架在经皮冠状动脉介入治疗中应用的有效性和安全性。方法经冠状动脉造影确诊为冠心病的162例患者术前给予控制血压、血糖、调脂、美托洛尔、硝酸酯类和ACEI类药物常规治疗,并使用阿司匹林300mg/d、泰嘉300mg/d的负荷剂量;术后继续使用阿司匹林100mg/d,泰嘉75mg/d作为维持剂量,其中泰嘉使用时间不少于1年。结果共置入支架240个,其中乐普支架158个、Excel支架82个,手术全部成功。所有患者术后随访,1例患者出院后3周自行停用泰嘉和阿司匹林,停用后1周发生猝死,其余患者无重要心血管不良事件发生。所有患者症状明显改善,胸痛缓解、活动耐量增加。术后半年40例复查冠状动脉CT成像2例出现再狭窄,26例复查冠状动脉造影2例出现再狭窄,总再狭窄率占置入总支架数的1.7%。结论泰嘉联合国产药物洗脱支架治疗冠心病安全,有较好的疗效,由于价格相对较低,符合我国国情,值得临床上推广应用。
